I've never seen a mental health provider before. Where do I begin?

Welcome — and congratulations on taking this first step. Starting with a psychiatric provider is simpler than most people expect. At MindSpring, you begin by scheduling an initial consultation through our secure online portal. From there, we handle the rest.

Your first appointment is a comprehensive evaluation — a relaxed, conversational session where Brian will listen to your history, your concerns, and what you hope to get out of care. Nothing is too small or too complicated to bring up. You'll leave with clarity and a starting point for your treatment plan.

What is a Psychiatric Mental Health Nurse Practitioner?

A Psychiatric Mental Health Nurse Practitioner is an advanced practice registered nurse with specialized graduate-level training in psychiatric and mental health care. PMHNPs are fully qualified to assess, diagnose, and treat mental health conditions — including prescribing and managing psychiatric medications.

Brian Macarthy is a board-certified PMHNP licensed in the state of New Jersey. His clinical background combines deep expertise in psychopharmacology with a holistic, whole-person approach to care — meaning you receive both the precision of expert prescribing and the warmth of genuinely compassionate care.

Do I need a referral from my primary care doctor to be seen?

In most cases, no — you can reach out to MindSpring directly without a referral. However, some insurance plans may require a referral for coverage purposes. We recommend checking with your insurance provider if that applies to you.

If your primary care physician has already completed screenings or evaluations, we welcome any records or documentation that help us understand your full picture — though they are never required to get started.

What ages do you treat?

MindSpring Behavioral Health provides psychiatric services for children, adolescents, and adults — covering a wide range of mental health conditions across the full lifespan.

Each age group receives care specifically tailored to their developmental stage, life circumstances, and clinical needs. For younger patients, we work closely with parents and caregivers to ensure treatment is effective, age-appropriate, and family-informed.

What conditions do you treat?

We treat a broad range of psychiatric and behavioral health conditions, including anxiety disorders, depression, bipolar disorder, ADHD, PTSD and trauma-related disorders, autism spectrum disorder, schizoaffective disorder, OCD and related conditions.

We also offer comprehensive psychiatric medication management for patients already engaged in therapy or other forms of treatment.

Are you currently accepting new patients?

The best way to check current availability is to reach out directly through our patient portal. Appointment slots vary, and we strive to accommodate new patients as promptly as possible.

What should I expect at my first appointment?

Your first appointment is a comprehensive psychiatric evaluation — typically 60 minutes. Brian will take time to truly listen to your story: your personal and family history, current symptoms, past treatments, lifestyle, and what you hope to achieve through care.

You will never feel rushed or pressured. By the end of the session, you'll have a clear sense of your diagnosis or working diagnosis, a recommended treatment direction, and next steps. Some patients leave with a prescription; others begin with a longer conversation before medications are introduced — it depends entirely on your individual situation and preferences.

How long are follow-up appointments?

Follow-up appointments are typically 20–30 minutes and focus on checking in about your progress, reviewing how medications are working, addressing any side effects or concerns, and adjusting your treatment plan as needed.

The frequency of follow-ups varies by patient. Early in treatment, we may meet more frequently, often every 2–4 weeks, to ensure your plan is dialed in. As you stabilize, visits may shift to monthly or quarterly.

Do you offer telehealth appointments?

Please contact our office directly for current information on telehealth availability. We are committed to making care as flexible and accessible as possible for all our patients in New Jersey.

What is your cancellation policy?

We ask that you provide at least 24–48 hours notice when canceling or rescheduling an appointment. Late cancellations or no-shows may be subject to a fee, as that appointment time is reserved specifically for you.

We understand that life happens. If something comes up unexpectedly, please contact us as soon as possible and we'll do our best to accommodate you.

What should I bring to my first appointment?

To make your first visit as smooth and productive as possible, please bring or have ready: photo ID and insurance card if applicable, a list of current medications, any prior psychiatric or medical records you'd like to share, a list of past medications you've tried and how they worked, your pharmacy name and location, and any questions or concerns you want to make sure we cover.

Do you accept insurance?

Please contact our office directly to discuss insurance and payment options. We strive to make quality psychiatric care as accessible as possible for all of our patients and are happy to clarify what's available to you.

What if I don't have insurance or my insurance doesn't cover mental health?

We encourage you to reach out regardless of your insurance situation. Our team can discuss self-pay options and will work with you to find a path that makes care feasible. Mental health support should not be out of reach.

Additionally, many insurance plans are now required to cover mental health services at the same level as physical health services under the Mental Health Parity and Addiction Equity Act — so it's worth checking your plan's details.

Do you provide Good Faith Estimates for uninsured or self-pay patients?

Yes. Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected charges before your appointment. If you are uninsured or paying out-of-pocket, please request this when you schedule your appointment and we will provide it in advance.

Will my mental health visits appear on my insurance records?

If you use insurance for your visits, your insurer will receive a claim that includes procedure and diagnosis codes — this is standard practice for all medical billing. If you are concerned about this, self-pay options allow you greater privacy since no claims are submitted to your insurer.

Will I definitely be prescribed medication?

Not necessarily — and never without your full understanding and consent. Medication is one tool among many, and it isn't always the right starting point for every patient or every condition.

During your initial evaluation, Brian will discuss all available options with you. Some patients benefit greatly from medication; others benefit more from therapy, lifestyle changes, or a combination. Every decision is collaborative and patient-led.

How long does it take for psychiatric medications to work?

It depends on the medication class. Antidepressants and mood stabilizers often take 2–6 weeks to reach their full therapeutic effect. Anti-anxiety medications and some others can work more quickly. Stimulants for ADHD typically have an effect within the same day they are taken.

Brian will give you realistic expectations for your specific medication and what to watch for. Regular follow-ups in the early weeks help us monitor your response and make adjustments if needed.

What if my medication causes side effects?

Side effects are common in the early stages of psychiatric medication, and most subside as your body adjusts. Brian will discuss potential side effects with you before starting any new medication and what to do if they occur.

If you experience troubling or unexpected side effects, reach out to us promptly — don't just stop a medication abruptly without guidance, as this can sometimes cause additional symptoms. We will work with you to adjust the dose, timing, or switch to an alternative if needed.

Can you take over management of medications I'm already on?

Yes, in many cases. During your initial evaluation we will conduct a thorough medication review — including your current prescriptions, dosages, how long you've been on them, and your experience with them.

We may request records from your previous provider to ensure continuity and safety. Medication transitions, when needed, are handled carefully and with your full understanding at every step.

How do I get prescription refills between appointments?

Refills are typically handled at or before your scheduled follow-up appointments. In some cases, we may send refills electronically between visits — this depends on the medication type and how you're doing.

Please do not wait until you've run out to request a refill. We ask that you contact us at least 5–7 days before you need your next supply so we have time to process it appropriately.

Do you provide therapy in addition to medication management?

MindSpring is a psychiatric practice, which means our primary focus is on evaluation, diagnosis, and medication management. While Brian incorporates therapeutic elements — psychoeducation, supportive counseling, and collaborative goal-setting — into every visit, we are not a therapy practice.

For patients who need or want ongoing therapy, such as CBT, DBT, EMDR, or talk therapy, we are happy to provide referrals to trusted therapists and maintain close collaboration to ensure your care is truly integrated.

Do you offer integrative or holistic mental health approaches?

Yes. While our care is firmly grounded in evidence-based psychiatric medicine, Brian also incorporates integrative and functional approaches where appropriate — including guidance on nutrition, sleep hygiene, exercise, stress management, and lifestyle factors that influence mental health.

Integrative approaches are offered as adjunctive support and are intended to complement — not replace — conventional psychiatric care. They are not always considered standard of care in conventional psychiatry.

Do you write letters for workplace accommodations, disability, or emotional support animals?

Requests for documentation — including workplace accommodation letters, FMLA paperwork, disability evaluations, or emotional support animal letters — are considered on a case-by-case basis and require an established patient relationship.

Please bring up any documentation needs during your appointment so we can discuss what is clinically appropriate and what we are able to provide.

What states do you serve?

MindSpring Behavioral Health currently serves patients in the state of New Jersey. Brian Macarthy is licensed as a Psychiatric Mental Health Nurse Practitioner in New Jersey, and all care is provided within that licensure.

Is everything I share with you kept confidential?

Yes. Confidentiality is a cornerstone of our practice and is legally protected by HIPAA. Everything you share in your sessions is private and will not be disclosed without your written consent, with very limited exceptions required by law.

Those exceptions include a credible risk of harm to yourself or others, suspected abuse of a child or vulnerable adult, or when records are subpoenaed by a court. Brian will explain these limits during your first appointment.

How are my records stored and protected?

All patient records are maintained through a secure, HIPAA-compliant electronic health record platform. Your personal health information is encrypted, access-controlled, and handled with the strictest professional standards.

A formal patient-provider relationship is only established after the completion of all intake procedures and clinical agreements through our secure portal.

Can I request my medical records?

Yes. You have the right to access your own medical records. Please submit a written records request through our office. We will process the request in accordance with HIPAA regulations, which typically allows up to 30 days for fulfillment.

Will you communicate with my other doctors or therapists?

Coordination of care is something we actively support — with your written consent. If you'd like us to communicate with your therapist, primary care physician, or other providers, we can share relevant clinical information to ensure your care team is aligned.

We will never share your information with other providers without your explicit authorization.

What should I do if I'm experiencing a mental health crisis?

Your safety is the top priority. If you are in immediate danger or experiencing a psychiatric emergency, call 911 or go to your nearest emergency room immediately.

You can also call or text the Suicide & Crisis Lifeline at 988. It is available 24 hours a day, 7 days a week, free and confidential.

MindSpring is an outpatient psychiatric practice and is not a crisis center. If you are between appointments and experiencing a mental health emergency, please use the resources above rather than waiting to hear from us.
